All adults attending camp in Texas (Trinity Pines, Lakeview Retreat Center and Austin College) are required by state law to complete the Child Protection Training for Texas Camps. A completion certificate for every person coming to camp over the age of 18 will need to be turned in upon arrival. Provide your adult leaders with the Ministry Grid link below to complete this training.

Each person attending Student Life Kids Camp must complete a Waiver and Release form, including all adult leaders. Group Leaders will receive a copy of the completed forms to their email address.
Lifeway is asking for a Certificate of Insurance (COI) for a Commercial General Liability policy from each church attending a Lifeway camp for 2023. This type of liability insurance covers the church from any claims made against your group due to your negligence while attending camp. Please click the link below for more information and a sample COI.
Parents will need some key information in order to complete the participant form. Fill-out and provide the following document at your next parent meeting to make completing the Waiver and Release form a breeze!
Each adult attending Student Life Kids Camp must complete Lifeway’s Child Protection Training course. Group Leaders will verify all adults have completed this course by signing the Church Agreement (above). This course is the ONLY approved curriculum for Student Life Kids Camp.
All participants (adults and kids) attending camp at Trinity Pines are required by the location to fill out an additional Trinity Pines-specific waiver. Distribute this packet to your group and bring physical copies to turn into Trinity Pines upon check-in. You do not need to bring physical copies of the Student Life Kids Camp waiver.

For 2021 camps, Lifeway's insurance provider requires us to collect proof of insurance from each church in the form of a Certificate of Insurance (COI) listing your church's camp dates. Contact your church's insurance agent and get a copy of the church's Certificate of Insurance to submit 30 days before camp to [email protected] or you'll have an option to attach it to the bottom of the Church Agreement form when you click on the SignNow link. Contact the Student Life Kids Camp office with any questions.
Please download the packet of forms below. You will only need to fill out the forms included in this packet. Please scan only completed Student Life Kids Camp Participant Forms (front and back) and send them in one large file to [email protected]. As required by the State of Texas, every adult leader attending camp at Lakeview must complete the DSHS Child Protection Training. You will need to provide Lakeview with proof of completion. Your Event Specialist, Sarah, will send instructions regarding training via email in late March.
