For the Kids Minister or Group Leader to complete and certify all adults have passed a background check.

CAMP PARTICIPANT FORM

Each person attending Student Life Camp must complete a Participant Form, including all adult leaders. The form can be signed electronically and does not require a notary stamp. Group Leaders will receive a copy of the completed forms to their email address.

Locations that require additional forms

All Texas Locations – Child Protection Training

All adults attending camp in Texas (Trinity Pines, Lakeview Retreat Center and Austin College) are required by state law to complete the Child Protection Training for Texas Camps. A completion certificate for every person coming to camp over the age of 18 will need to be turned in upon arrival. Provide your adult leaders with the Ministry Grid link below to complete this training.

All participants (adults and kids) attending camp at Trinity Pines are required by the location to fill out an additional Trinity Pines-specific waiver. Distribute this packet to your group and bring physical copies to turn into Trinity Pines upon check-in. You do not need to bring physical copies of the Student Life Kids Camp waiver.

Southeastern University

In addition to the Student Life Kids Camp Participant Form, Southeastern University requires this Minor Swim Permission Waiver for all minors wanting to use the swimming pool. Please advise parents to enter “Student Life Kids Camp” as the camp name.
